3) The first session will consist of a thorough history, including baseline measurements and initiation of treatment. We will discuss your goals and the most effective treatment plan for you. Once we have a plan in place, including an independent home program, we will discuss the frequency and duration that is most appropiate for you.

How is it paid for?
Activestyle PT is an “out of network” provider. The patient is expected to pay for services at the time of visit. Activestyle PT will provide you with a “superbill”/invoice that you can submit to your insurance for reimbursement. Please call your insurance company prior to your visit for information regarding your “out of network” benefits. Attached is a worksheet containing appropriate questions to ask the customer service representative.
Why is Activestyle PT out of Network and what does this mean?
“Out of Network” means the therapist has not entered into a contract with individual insurance companies to receive reimbursement based on their contracted rates. Each insurance company has their own contracted rates and regulations, and Activestyle’s energy is best spent working with patients. With an “out of network” model, you will have one on one services with your provider. This model of care will allow for more individualized care and longer treatment sessions resulting in fewer overall visits.
What steps are involved in submitting a claim to my insurance company?
The process is quite simple. Activestyle PT will provide you with a “superbill”/invoice at the time of service and you may submit that invoice to your insurance company for reimbursement. The invoice has all the necessary information (business name and address, Tax ID, National Provider Number, license numbers, as well as the patient’s ICD-9 (diagnosis) and CPT (billing) codes. You may choose to submit bills following each visit or one time per month.
What form of payment do you accept?
We accept cash, check, debit, credit cards as well as Health Savings Account (HSA) and Flexible Spending Account (FSA). HSA and FSA cannot be used for wellness and Pilates sessions.
What if I need to cancel?
We understand that life happens and we strive to be as flexible as possible. That is why our cancellation policy results in no charge if appointments are cancelled with 24 hours or more notice. If a scheduled appointment is cancelled within 24 hours of the appointment, a $35 charge will occur. “No Shows” for appointments will incur a full visit charge.
What equipment do I need to have for a telehealth visit?
A desktop, laptop, tablet or smartphone with internet connection is needed to conduct the telehealth visit. Activestyle PT uses a HIPPA compliant medical software to conduct telehealth visits.
What type of space do I need for my treatments?
You will need enough space for a treatment table to allow for an evaluation and treatment. We will provide all the supplies and equipment needed for your session.
Do I need to see a doctor prior to starting physical therapy?
No, New Hampshire and Massachusetts are direct access states. You are allowed to see a Physical Therapist without a referral from a doctor. However, some insurance companies require a referral to be submitted with your claim to obtain reimbursement .
Do you communicate with my physician?
Yes! Our goal is to provide the best care possible to all our clients, which may include disussion with your physican. Any information shared with your provider(s) will require a written release per HIPAA guidelines.
What is your COVID-19 policy?
Activestyle Physical Therapy follows all CDC recommended guidelines and precautions. You will be asked to answer COVID-19 screening questions before each treatment. All products will be disinfected prior to being brought into your home as well as after each session. A face mask will be worn by your therapist and we ask that you kindy wear a mask for the duration of each session.
When should I start physical therapy?
As soon as possible. Research shows the sooner you see a PT the faster the recovery and the quicker you will return to your previous activity. Being proactive in your care will ultimately lead to less healthcare costs.
How many visits will I need?
It depends on your goals. Each session provides education, hands on treatment as well as an individualized exercise program. We will efficiently and effectively work towards your short-term and long-term goals.
If you are looking for structure, accountability and/or have multiple goals, you may require more visits. If you are looking for one, or minimal sessions to develop a plan to independently implement, we are here to help you!
What happens after my Physical Therapy is completed?
After all your sessions are completed, your Physical Therapist will make sure that you have a comprehensive home exercise program to continue with for the final stages of your rehabilitation.
